Longevity Gyms is a precision wellness facility located at 7030 Hi Tech Dr, Suite 102, Hanover, MD 21076. We are operated by IFHO and offer a range of science-backed therapeutic modalities including Hyperbaric Oxygen Therapy, PEMF, IV Therapy, Vasper, EWOT, Flowpresso, and others.

Health & Medical Information

Important: We collect health and medical information through our intake forms to ensure your safety during sessions and to develop effective wellness protocols. This information is treated with the highest level of care and confidentiality.

Health intake forms are collected before your first session and include information about medical history, current medications, recent surgeries, implants, contraindicated conditions, and wellness goals. This information is used exclusively for:

  • Determining which modalities are safe and appropriate for you
  • Customizing session settings and protocols to your needs
  • Informing our team of any conditions requiring special attention during your session
  • Building your personalized wellness protocol over time

We do not share health intake information with insurance companies, employers, or third parties for marketing purposes. Your health information is accessible only to the Longevity Gyms team members who directly administer your care.

Please note: Longevity Gyms is a wellness facility, not a HIPAA-covered healthcare entity. While we protect your health information with care and discretion consistent with industry best practices, standard HIPAA protections do not apply to wellness facility-client relationships.

  • Health intake forms — retained for 5 years from your last session, or as required by applicable Maryland health and wellness regulations

  • Marketing opt-in records — retained until you opt out, plus a reasonable period to process the opt-out
  • Website analytics data — retained in anonymized form per the terms of the analytics service used

When we no longer need your information, we delete or anonymize it securely. You may request earlier deletion of your data subject to the limitations in Section 09.

Deletion

We will inform you of any exceptions at the time of your request.

Opt-Out of Marketing

You may opt out of marketing emails at any time by clicking the "Unsubscribe" link in any email we send, or by contacting us directly. You may opt out of SMS marketing by replying STOP to any text message from us. Opting out of marketing will not affect transactional communications related to your bookings or membership.

Minors may receive services only with explicit parental or guardian consent and in-person supervision, as required by the contraindication protocols for each specific modality.
